In Paris, 1902, Pierre Robin introduced the “mono-block” as an oral appliance which allows the re-establishment of the normal relationship between the mandible and maxilla in children with hypoplasia of the mandible. Normal dental occlusion became possible and those children were able to breathe through their nose again.

 

This “mono-block” appliance was used until 1985 to treat patients with severe obstructive sleep apnea and snoring.

 

As of today, over 55 different Mandibular Advancement Appliance (MAA) have been released in the market.

 

The Mandibular Advancement Appliance (MAA) has several benefits; it is easy to use, comfortable, unobtrusive, portable and non-surgical. It costs is less than the surgery and the treatment does not require any hospitalization. Thus, it is the most efficient and tolerant treatment of snoring and obstructive sleep apnea. 

Inserted intra-orally at night, it:

  • Displaces the position of the mandible anteriorly

  • Tenses the tongue 

 

 

Hence, by these mechanisms, it widens the upper respiratory space and reduces the degree of pharyngeal collapse. These observations were confirmed by a number of imaging studies using video-endoscopy, computed tomography and magnetic resonance imaging (MRI).

 

The American Sleep Disorders Association (ASDA) reviewed the available literature and recommended that oral appliances should be used in patients with primary snoring or mild OSAS and in patients with moderate to severe OSAS who are intolerant of, or refusing treatment with, ventilation. Also, MAA appears to be an effective therapy for treating persistent apnea in patients whose surgeries failed.
